We’re actually doing, which makes it so much easier to do when you actually go to the competition.”&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-family: &amp;quot;Trebuchet MS&amp;quot;, sans-serif;"&gt;The class spent one semester doing research for Contemporary Cosmetic Surgery, a local company that asked the class to help them reach the teen market.&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-family: &amp;quot;Trebuchet MS&amp;quot;, sans-serif;"&gt;Students organized focus groups and conducted their own research about teens’ attitudes about tanning, which they presented to the company.&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-family: &amp;quot;Trebuchet MS&amp;quot;, sans-serif;"&gt;“The students did a wonderful job,” said Dr. Brian Michelow of Contemporary Cosmetic Surgery in a press release. “We learned that teens are hearing the message about healthy skin but they are not necessarily listening. Mrs. Getz and her class helped us understand the teen thought process when it comes to skincare and what was important to them in product and services."&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-family: &amp;quot;Trebuchet MS&amp;quot;, sans-serif;"&gt;In fact, the company was so pleased with the students’ work, they paid them double.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family: &amp;quot;Trebuchet MS&amp;quot;, sans-serif;"&gt;The class teaches the students more than just marketing. From how to act and dress around a real business client to the vocabulary the students have to decipher from the college-level textbook the class uses, the students walk out more prepared for the real world, they said.&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-family: &amp;quot;Trebuchet MS&amp;quot;, sans-serif;"&gt;With the after-school internships they are required to hold during the school year, the independent work outside class with their client and even the fact that they choose their own curriculum for the school year, responsibility plays a huge role for Getz’s class.&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-family: &amp;quot;Trebuchet MS&amp;quot;, sans-serif;"&gt;The rest of this school year, the class will continue refining their research to send it to a state legislator who has proposed a bill that would more strictly prohibit tanning for minors. They will also continue Chik-Fil-A breakfasts they have sporadically throughout the year, and they are selling T-shirts to raise extra money.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family: &amp;quot;Trebuchet MS&amp;quot;, sans-serif;"&gt;The class is part of Excel TECC, a six-school collaboration that offers college prep, tech prep and other learning opportunities to students. Other classes in Excel TECC include the Culinary Arts class, which runs a café in Beachwood High School’s cafeteria; and the Green Dream, which hosts a green business expo annually. During this clinical rotation, Dr. Michelow was generous in sharing information about clinical techniques, patient interactions, and professional demeanor. &lt;br /&gt;&lt;p&gt;It was beneficial to have to opportunity to first assist during most surgery cases. Having the opportunity to work closely with Dr. Michelow has allowed me to improve my clinical skills such as suturing, various injections, and incision and drainages. &lt;br /&gt;&lt;p&gt;&lt;br /&gt;Surgeries ranged from extensive reconstructive and cosmetic cases in the operating room to various in-office procedures. It was interesting to work with a patient population ranging in age from toddlers to the elderly and being involved in surgery cases dealing with every part of the body from head to toe. &lt;br /&gt;&lt;P&gt;&lt;br /&gt;Dr. Michelow left me inspired to reach further and do more in my career as a PA. Michelow, MD, FACS.</name><email>noreply@blogger.com</email><gd:image rel='http://schemas.google.com/g/2005#thumbnail' width='29' height='32' src='http://2.bp.blogspot.com/-yXjU2DhDlpA/TayP8RRK09I/AAAAAAAAAAY/uxKdZ_sb9Vk/s220/BJM_W1Photo.jpg'/></author></entry><entry><id>tag:blogger.com,1999:blog-3718054529048946758.post-4805188232714463053</id><published>2011-02-09T01:01:00.004-05:00</published><updated>2011-02-09T01:17:48.742-05:00</updated><category scheme='http://www.blogger.com/atom/ns#' term='Breast Implants ALCL Cancer'/><title type='text'>Breast Implants are in the News again - What is ALCL?</title><content type='html'>The FDA recently released a communication to health care providers and the public about a rare cancer of the immune system termed anaplastic large cell lymphoma (ALCL).&lt;br /&gt;&lt;br /&gt;Research indicates that women with breast prostheses may have a very small but increased risk of developing this disease in the scar capsule that develops arround the implant.&lt;br /&gt;&lt;br /&gt;&lt;span style="color:#3333ff;"&gt;&lt;em&gt;&lt;strong&gt;Is this common?&lt;/strong&gt;&lt;/em&gt;&lt;/span&gt;&lt;br /&gt;The number of cases is small (estimated at 60) compared to the total number of women with breast implants (5 to 10 million) world wide.&lt;br /&gt;&lt;br /&gt;&lt;span style="color:#3333ff;"&gt;&lt;em&gt;&lt;strong&gt;For women with implants:&lt;/strong&gt;&lt;/em&gt;&lt;/span&gt;&lt;br /&gt;The FDA recommends that women do not need to change their routine medical care.&lt;br /&gt;&lt;br /&gt;Monitoring of the implants by a health care provider at regular intervals is recommended.&lt;br /&gt;&lt;br /&gt;Among the confirmed cases of ALCL are patients with silicone as well as saline prostheses.&lt;br /&gt;&lt;br /&gt;&lt;span style="color:#3333ff;"&gt;&lt;em&gt;&lt;strong&gt;What to look for:&lt;/strong&gt;&lt;/em&gt;&lt;/span&gt;&lt;br /&gt;After the breasts have healed from the augmentation surgery, look for any changes in one or both breasts.&lt;br /&gt;There may be new onset of pain, swelling, lumps or firm nodules that develop around the prostheses.&lt;br /&gt;&lt;br /&gt;&lt;span style="color:#3333ff;"&gt;&lt;em&gt;&lt;strong&gt;For Health Care Providers:&lt;/strong&gt;&lt;/em&gt;&lt;/span&gt;&lt;br /&gt;Because ALCL has generally only been identified in patients with late onset of symptoms such as pain, swelling (seroma) , lumps (capsular masses), the FDA does &lt;span style="color:#000000;"&gt;not&lt;/span&gt; recommend prophylactic breast implant removal in patients without symptoms or other abnormality.&lt;br /&gt;&lt;br /&gt;&lt;span style="color:#3333ff;"&gt;&lt;em&gt;&lt;strong&gt;What to test for when suspecting ALCL:&lt;/strong&gt;&lt;/em&gt;&lt;/span&gt;&lt;br /&gt;If the decision has been made to remove the prostheses then the following test are recommended by the FDA:&lt;br /&gt;&lt;br /&gt;1. Fresh seroma fluid and representative portions of the capsule should be collected and sent for pathology tests to rule out ALCL.&lt;br /&gt;&lt;br /&gt;2. Diagnostic evaluation should include:&lt;br /&gt;     a) Cytological evaluation of seroma fluid with Wright Giemsa stained smears and&lt;br /&gt;     b) Cell block immunohistochemistry testing for cluster of differentiation (CD) and&lt;br /&gt;         Anaplastic Lymphoma Kinase (ALK) markers.&lt;br /&gt;&lt;br /&gt;3. The following is a brief summary of those results.&lt;br /&gt;&lt;br /&gt;32 attendees, 28 returned surveys&lt;br /&gt;Survey questions 12&lt;br /&gt;Presenter variables 1,568&lt;br /&gt;Total data points 6,832&lt;br /&gt;&lt;br /&gt;96% felt the CEU’s offered were an important reason for attending the course.&lt;br /&gt;96% said they would return if a course were offered here next year.&lt;br /&gt;100% said they would recommend this course to a colleague.&lt;br /&gt;&lt;br /&gt;Neuro rehab, shoulder rehab, tendon injuries, kinesiotaping were mentioned as topics attendees would be interested in learning more about.&lt;br /&gt;Attendees were satisfied with the quality of the presentations.&lt;br /&gt;&lt;br /&gt;We are currently discussing which topics would be most popular while still lending itself to the expertise of our staff, should we decide to offer another workshop.  All involved in presenting this workshop agreed that while it was a difficult, time intensive endeavor it also proved to be a rewarding learning experience. Students who show interest in marketing are selected to participate in the class. As&lt;br /&gt;a team, they choose the company for which they would like to perform research and&lt;br /&gt;develop a marketing plan.&lt;br /&gt;&lt;br /&gt;I am honored to announce the class has chosen Contemporary Cosmetic Surgery and our&lt;br /&gt;products for their current research and promotion develop program..&lt;br /&gt;At an introductory meeting with the students last week, we proposed the following&lt;br /&gt;challenges:&lt;br /&gt;&lt;ol&gt;&lt;li&gt;How do we promote healthy skincare habits and the use of sunscreen to teens&lt;br /&gt;and young adults? &lt;/li&gt;&lt;li&gt;How do we get the word out that melanoma is the second most common cancer&lt;br /&gt;affecting teens? Protecting the skin with sunscreen is a simple and effective means&lt;br /&gt;of preventing Melanoma. &lt;/li&gt;&lt;li&gt;Acne is a teens nemesis. How can a practitioner educate teens about the&lt;br /&gt;management of a serious condition that may lead to scarring,&lt;br /&gt;disfigurement and personal embarrassment. &lt;/li&gt;&lt;/ol&gt;&lt;p&gt;&lt;br /&gt;We are looking forward to the recommendations this bright group of teenagers will&lt;br /&gt;provide. A special thank you as well, to all the Exhibitors that supported this event.&lt;br /&gt;&lt;br /&gt;The 2010 Conference of the OVSPS is now behind us. Feedback from the attendees has been uniformly positive. Many participants stated that the caliber of the meeting matched that of national and international conferences.&lt;br /&gt;&lt;br /&gt;The theme of our conference was: "From laboratory research to clinical reality." This theme was well represented by the panel discussion regarding the form-stable cohesive silicone gel breast prosthesis by the Plastic Surgery researchers at Cleveland Clinic Foundation. The practical nature of the presentations and the vast experience of the presenters were well received. We acknowledge the commendable support provided by the Exhibitors for all their research contributions.&lt;br /&gt;&lt;br /&gt;I am pleased to mention that there were a number of firsts this year. There were a record number of abstracts submitted for presentation by residents, fellows and medical students. Although this was the 53rd Annual Conference, posters were displayed for the first time. Judging of the posters was done during the evening cocktails on Friday and provided a relaxed yet entertaining education opportunity for all. The social events on Saturday evening included live jazz entertainment during a delicious dinner at the Lakefront Science Museum that was climaxed by the spectacular IMAX Hubble show.&lt;br /&gt;&lt;br /&gt;I mention these facts, not to pat the Program Committee on the back nor to blow my own horn but rather to stress how the support (both professional and financial) of our members and the exhibitors ensured the success of our meeting.&lt;br /&gt;&lt;br /&gt;The program committee did the research to set up the conference but it was contributions and support from the members and the Exhibitors that made it a reality. Dr. Michelow offers three techniques to slough off dead skin cells to promote collagen production, eliminate fine lines and wrinkles and addres acne. These techniques are microdermabrasion, peels and&lt;br /&gt;dermaplaning. &lt;br /&gt;&lt;br /&gt;While all three techniques offer glowing results, they address different skin conditions.  Dr. Michelow's expert estheticians&lt;br /&gt;will evaluate your skin and discuss your goals and then recommend techniques that will work best for you.  While improvement in your skin's appearance may be immediate, you may acheive the most optimal results with a series of treatments and of course a proper skincare program at home. &lt;br /&gt;&lt;br /&gt;&lt;span style="font-size:130%;color:#990000;"&gt;&lt;strong&gt;Microdermabrasion:&lt;/strong&gt;&lt;/span&gt; Dr. Michelow's estheticians offers crystal free mechanical microdermabrasion. A stainless diamond tipped steel wand is gently glides over your skin. Gentle suction draws up dead skin cells and polishes the skin. Our red light therapy included with treatment stimulates collagen production and is anti-bacterial to promote healing.   There is no down time and no pain. Most clients experience a pleasant "tingle."  This quick "lunchtime" procedure will leave your skin glowing.  Service is finished with an application of sunscreen. There is little to any redness. There are many benefits to microdermabrasion. &lt;br /&gt;&lt;br /&gt;Microdermabrasion can addresss stage one acne (black heads and comedones),&lt;br /&gt;hyperpigmentation in clients of any color, signs of aging and photo damage and can treat minor and superficial scars.   &lt;br /&gt;&lt;br /&gt;&lt;strong&gt;&lt;span style="font-size:130%;color:#990000;"&gt;Dermaplaning:&lt;/span&gt;&lt;/strong&gt;  May only be performed in a physicians office.  A trained esthetician will slough off dead skin cells with a stainless razor tipped tool.  It is appropriate for all patients beginning in the late 20s.  Dermaplaning encourages the production of new cells, assists with the absoption of skincare products, removes minor wrinkles and improves skin texture. &lt;br /&gt;It is effective with acne scaring. May be performed on clients of any skin of  any color.&lt;br /&gt;&lt;br /&gt;&lt;span style="font-size:130%;color:#990000;"&gt;&lt;strong&gt;Mild Peels:&lt;/strong&gt;&lt;/span&gt;  Alphahydroxy acids (AHAs), such as glycolic, lactic, or fruit acids are the mildest of the peel formulas and produce light peels. These types of peels can provide smoother, brighter-looking skin for people who can't spare the time to recover from a phenol or TCA peel. AHA peels may be used to treat fine wrinkling, areas of dryness, uneven pigmentation and&lt;br /&gt;acne. Various concentrations of an AHA may be applied weekly or at longer intervals to obtain the best result. Your doctor will make this decision during your consultation and as the treatment proceeds. "Just block and stop," says licensed aesthetician, Medical aesthetician, Gena O'Neill.&lt;br /&gt;&lt;br /&gt;"Blocking refers to using either sun block or sunscreen and by blocking smoking, I mean giving up cigarettes," says Gena.&lt;br /&gt;&lt;br /&gt;How is it that something like the sun that feels so good and improves our moods can cause damage? "There are three types of UV rays in the atmosphere; UVA, UVB, UVC. UVC are absorbed into the environment," says Gena, "UVA rays age the skin, (A equals age) and UVB (B equals burning) are harmful to our skin. UV A rays take longer to impact the skin but penetrate deeper. UVA rays results in wrinkles and discoloration and the UVB results in burns. A tan is the skin's attempt to protect itself against these damaging rays."&lt;br /&gt;&lt;br /&gt;So why should we care? Over exposure to the sun can lead to serious consequences that are worse than aging skin. The rays can cause skin cancer. Melanoma is the most serious type of skin cancer and can be lethal. Melanoma can spread throughout the body. Basal cell cancer's don't spread but can erode the underlying structures of the skin and require reconstructive surgery and squamous cell cancers can also spread through out the body.&lt;br /&gt;&lt;br /&gt;This means everyone; even those with darker skin tone must protect their skin.&lt;br /&gt;&lt;br /&gt;Protection comes in the form of a sun block or a sunscreen. Gena explains the difference:&lt;br /&gt;"A sun block offers complete protection of the skin. It is a lotion that blocks the rays of the sun. Its ingredients include zinc oxide and titanium dioxide. Sunscreen protects the skin by reflecting and absorbing harmful UVA and UVB rays."&lt;br /&gt;&lt;br /&gt;"Look for broad spectrum protection" says Gena. Sunscreen contains benzophenones which protect against UVA rays and cinnamates and salicylates that work against UVB rays. SPF stands for Sun Protection Factor. It is recommended to use products with at least SPF 15. The higher the SPF the longer it acts in the sun without breaking down. Sunscreens need to be reapplied.&lt;br /&gt;&lt;br /&gt;"If you smoke, stop," says Gena. Smoke robs the skin of oxygen and its youthful texture. Wrinkles form around the lips, eyes and mouth do to sucking. A 2007 study showed that even photoprotected skin ages more rapidly in a smoker.&lt;br /&gt;&lt;br /&gt;Sun damage to skin cannot be reversed but it can be treated. Many women are turning to dermal fillers to achieve the lush lips of &lt;strong&gt;Angelina Jolie&lt;/strong&gt;. Other women are looking to restore the pillowy appeal of youthful lips.&lt;br /&gt;&lt;br /&gt;Lately, the tabloids are having field day featuring celebrities with overstuffed lips and "bad" plastic surgery in general. My patients have been asking me if there is any aesthetic formula used to create full lips that enhance rather than overtake the face.&lt;br /&gt;&lt;br /&gt;The answer is yes. In most circumstances the width of the lower lip should be two-thirds wider than the upper lip. I keep this in mind when appraising all patients regardless of their age. There should be a pleasing upward curve to the lips, giving the impression of a smile rather than a scowl. I work with the natural architecture of the lip to accent the cupids bow.&lt;br /&gt;&lt;br /&gt;Patients who are concerned about aging should keep in mind that as gravity takes over, the face and it features begin to sag. That is why we use fillers to lift in addition to filling in lines. As time goes by the vertical lines between the nose and the upper lip begin to flatten.
